Intake Nurse (LPN or RN)
POSITION RESPONSIBILITIES
- Identify and evaluate client needs and determine appropriateness for personal care services.
- Upon receiving a referral, make an initial phone call immediately. If unable to call immediately, the initial call must be completed before the end of the same business day.
- Complete intake forms upon client admission.
- Notify the Office Manager and Scheduling Team upon completion of the intake.
- Intake must be completed within 48 hours of receiving the referral.
- Admission paperwork must be completed within 24 hours of the intake.
- Communicate routinely with the Office Manager and Scheduling Team regarding client-specific needs and requests.
- Complete all intakes for Medicaid and/or PASSE clients discharged from a hospital or facility within 48 hours of the client returning home.
- Ensure all required supporting documentation for reassessments is completed and filed in the client's chart.
- Complete Supervisory Home Visits according to the following schedule:
- Medicaid/PASSE: Annually
- AR Choices: Every 6 months, alternating visits with the aide present and not present
- VA: Monthly or as specified by authorization
- Complete all Prior Authorization (PA) paperwork for the initial visit as directed by the Office Manager or Director of Nursing (DON).
- Enter PA requests into the appropriate payer-source portal and update the Office Manager once submitted.
- Develop the Care Plan following the initial assessment in consultation with the client and/or responsible party.
- Document and report any changes in client status to the Office Manager immediately.
- Review and update Care Plans in accordance with company policy.
- Participate in community-related functions as requested by the Office Manager and/or marketing staff, schedule permitting.
- Assist with PCA courses as requested by the DON.
- Attend and actively participate in weekly staff meetings.
- Perform other duties as assigned by the Office Manager, DON, and/or Director.
QUALIFICATIONS
- Current Arkansas LPN or RN license in good standing.
- Clear criminal background screening.
- Home care experience preferred.
- Computer literate with the ability to maintain simple records.
JOB CONDITIONS
- Regular office hours: 8:00 AM to 4:00 PM, Monday through Friday.
- Position requires meeting deadlines and can be stressful at times.
- Primarily a desk position involving:
- Sitting
- Standing
- Stooping
- Walking
- Extensive telephone communication
- Requires minimal lifting of office records and printed materials.
- Ability to read 12-point font or larger.
- Must be able to hear adequately on the telephone (with amplification if needed) and communicate effectively in English, both verbally and in writing.
- Requires travel to client homes and occasional travel for in-service training.

The position requires the ability to operate:

- Multi-line telephone system
- Other standard office equipment as needed
